myProfile tracks your summary health data & personal info.
Upon registration you will notice your first and last names
are already published and will carry throughout your online
system record pages.

Within this page (as per other recorded pages), you can
upload files and/or enter important data concerning your
personal health. Users are limited to following file formats:
(.pdf jpg .gif .doc) only.

For efficiency purposes, your pages also track the last time
you revised your records. (see top right corner of record pages)

myProfile provides a benefit by offering you and your health care professional a quick summary report of the status of your current
health situation, emergency telephone contacts... including
your health insurance and/or agent identification.
